Her rheumatologist instructed her to undergo a baseline ocular examination prior to initiating Plaquenil (hydroxychloroquine, Sanofi-Aventis) therapy. Toxic maculopathy associated with chloroquine use was first documented in the literature five decades ago.1 In the United States, Plaquenil––an analog to chloroquine––is used to treat a variety of conditions, including rheumatoid arthritis, lupus and several distinct inflammatory disorders. Although the incidence of macular toxicity is infrequent with Plaquenil use (at a dosage of 200mg or 400mg q.d.), its visual impact can be devastating.

An eye exam is recommended at the start of treatment to detect any existing vision problems; follow-up tests are needed to catch retinal damage in the earliest stages, before symptoms appear. If found later, damage to the eye may be irreversible, even if the medication is stopped. Plaquenil can cause serious vision problems that aren't always reversible. Tell your doctor immediately if you notice any changes in vision or if you've ever had vision changes while taking. Plaquenil hydroxychloroquine is in a class of drugs called disease-modifying anti-rheumatic drugs, which are used to decrease inflammation, pain and joint damage. While today it is used to treat autoimmune conditions, such as rheumatoid arthritis and lupus, it was originally used as an anti-malaria drug.

Initially, central visual acuity may be unaffected, but the patient may notice related paracentral scotomas that often interfere with reading. The associated classic retinal toxicity is described as a bull’s eye maculopathy (ring of depigmented retinal pigment epithelium that spares the foveal area).

The Plaquenil Problem “Plaquenil toxicity is very distinctive,” said Michael F. Marmor, MD, professor of ophthalmology at Stanford University. Dr. Marmor, who chaired the Academy’s screening guidelines committee, said the goal in Plaquenil screening is to catch changes at a very early stage when there’s just a minimal amount of loss of vision. Plaquenil is a drug used to treat Rheumatoid Arthritis and Systemic Lupus Erythematosis. It is a very good drug and gives many patients significant relief from their symptoms. Plaquenil related eye complications are not common. Plaquenil can produce pigment changes in the macula of the retina. I was just talking to my rheumatologist today. She has prescribed plaquenil. She said very rare side effect is some sort of deposits on the eye. I would contact your dr to be on safe side. My dr said to have eye exam with dilation a few months after starting. Just tell eye dr you take plaquenil and she said they would know exactly what to look for.

Are Longer-term Antibiotics More Effective than Shorter-term. In this randomized, double-blind trial involving patients with persistent symptoms attributed to Lyme disease, prolonged antibiotic treatment ceftriaxone followed by 12 weeks of either doxycycline or clarithromycin–hydroxychloroquine did not lead to a better health-related quality of life than that with shorter-term treatment ceftriaxone followed by placebo,” the authors concluded.
